COMPREHENSIVE TRAIL PLAN
The proposed development will construct a section of ten -foot -wide bituminous trail
starting at the southwest intersection of Lyman Boulevard and Great Plains Boulevard
traveling south for approximately 675 feet to the southern terminus of the property.
Future development activity to the south will extend the trail to a connection point at
the Bandimere Park pedestrian underpass. The planned street and sidewalk plan must
provide convenient pedestrian access from each of the homes to the planned and in-
place pedestrian improvements at the intersection of Lyman Boulevard and Great
Plains Boulevard.
Recommended Conditions of Approval
1. Full park dedication fees shall be collected per city ordinance in lieu of requiring
parkland dedication, at current park fee rates this fee would total $121,800; and
2. Construction of Great Plains Boulevard trail from the southwest intersection of
Lyman Boulevard and Great Plains Boulevard traveling south for approximately
675 feet to the southern terminus of the property. The developer shall provide
design, engineering, construction and testing services required of the Great Plains
Boulevard trail. All construction documents, including material costs, shall be
delivered to the Park and Recreation Director and City Engineer for approval
prior to the initiation of construction. The trail shall be 10 feet in width, surfaced
with asphalt and constructed to meet all city specifications. The applicant shall be
reimbursed by the City for the cost of the aggregate base, trail surfacing, and
storm water systems utilized to construct the trail. This reimbursement payment
shall be made upon completion and acceptance of the trail and receipt of an
invoice documenting the actual costs for the construction materials noted. Labor
and installation, design, engineering and testing services are not reimbursable
expenses.

Reach for Resources, Inc. has moved our administrative office to Minnetonka,
Minnesota. Reach had been within the Eisenhower Community Center in
Hopkins, Minnesota for thirty years. We are saddened to leave our colleagues an
friends within the Hopkins School District but we know that we will continue to
be close neighbors.
Each year, Reach supports about 1,000 children and adults through mental
health services, community living supports, employment services, inclusion, and
therapeutic recreation. Currently, we are expanding a couple of our programs
while some others are seeing steady growth; so Reach is growing. In addition,
employees are able to work more mobile than ever before and we need efficient
office space for employees when they work within the office space.

Your favorite things about working for Reach/Your favorite story: There are so many things that I love about
Reach, however, the thing that I love most of all are the conversations that I have with theartici ants. I am

always excited to go to programs and to be able to talk, laugh and dance with all of them. They Y g truly brighten
my day. I have so many great memories of Reach; it is hard to pick just one. However, one of m favorite

memories at Reach was when I was spending time on the youth lane at bowling. The decided one articular

day to play Name that Tune and sing me their favorite songs. The p caparticipants ended u calling me old

because I didn't know any of them! I never knew that bowling could be so musical.
What I will miss: There are many things that I will miss at Reach. I will miss my coworkers and the knowledge
and support that they have given me throughout my time here. But most of all I will miss the participants.
Whether it is watching them try something for the first time, witnessing friendships that they form between
each other, or when they call just to tell me their bowling score. They never cease to keep me one my toes!

I have enjoyed forming relationships with the families I serve, affecting change, and seeing s grow. row. I am

I will of course miss the Reach staff who have helped pave the path that I am leading today by teaching me
valuable skills and creating opportunities I would have never gotten anywhere else. But, the thin that will

Reach for Resources has always been proud to see their work and clients succeed, so much that
we want to share some of our stories with you. Here are just a few of our many recent success
stories of our hard working and respected clients.
Jack:
Jack is an eztremelu fun person to spend time with. It is rare to meet someone who is as positive and excited about life
as he is. Jack has been working on his health and fitness with staff and is committed to keeping up his healthq lifestq1e.
Jack likes to walk on the treadmiE and participate in Zumba class at his local gum. There are a lot of people who know him
at the gum and he is such a positive influence on everyone there. Jack has also excelled with his social skills, understanding
appropriate social interaction and knowing which personal information should be kept private and not shared with others.
Jack's presence at the gqm is a great asset to his community.

Over the last few months, Ando (who is alwaus a v¢rq determined and hardworking man) has reallq outdone himself. With just a little staff
encouragement, Andu has completelu overhauled his apartment. Andu has found new pride in organizing and decorating his home, and often
sets projects for himself and staff to complete. Andu is verb pleased with the progress he has made in his home, but is showing no signs of
slowing down anu time soon! Not onlu has Andu made remarkable strides in his home, but he has also taken his fitness to a whole new level.
Each daq Ando looks to set new, higher goals for himself and he works veru hard to achieve and exceed them. He looks forward to learning about
different aspects of fitness and exercise, and enjous implementing them in his dailq routine.
Andu is a remarkable man, and through his efforts and accomplishments he is continuousluinspiring those around him.
A shleu:
AshleU started receiving Children's Mental Health Case Management through Reach for Resources almost two Uears ago. RecentlU, AshleU was
evicted from her apartment and left with nowhere to lige due to some hardships with her roommates. With help from her supportive aunt and
j case manager, AshleU was able to obtain her verb own apartment. While shelter was taken care of, Ashleu still needed furniture. Her case
manager was able to connect Ashleq with Briding and scheduled an appointment for her to get the items she needed. During the cold month
of Februarq, AshleU was able to "warmup" her apartment bq going "shopping" at Bridging for what she needed to furnish her apartment.
On the daq of the appointment, Ashleu had fun and was veru happq with the furniture and household items she was able to now claim as
"mine." Ashleu, with the support of her case manager, was able to select items including recliners, tables, dishes and other kitchen items that
fit her stale. Ashleu was excited to have all of her "new" things delivered to her apartment and grateful for the assistance and support received
from her case manager. Ashleu loves her furniture and living in her own space. She even asked her aunt and cousin to help her rearrange it
so everything fit well and looked good for her guests! Both Ashleu and her aunt appreciate the services she has received through Reach for
Resources and for the assistance in connecting with Bridging.

On the Town0On Town is open to adults 18 and older who have done an intake and been accepted into the program. Individuals may sign up
for each On the Town activity separately. Each activity costs $5 to register plus any additional fees (such as tickets, transportation,
