NOW, THEREFORE, BE IT RESOLVED that the Chanhassen City Council hereby approves Change Order #4 for the lower level remodeling project in the amount of $26,948.00, for Gen Con Construction, and $6,395.00 for KKE Architects. Passed and adopted by the Chanhassen City Council this 14th day of June 2004. These proposals cover a e variety of work items described below: ProDosal ReQuest No.1 The doors for the new restrooms were installed in the last remodel in a location that does not allow current accessibility codes to be met. These doors need to be removed from their present location and re-set. Cost $375.00 ProDosal ReQuest No.2 This request for additional work was generated by staff to accomplish the following work items: 1. New floor tile to replace the asbestos tile and new tile in the expanded "tech" room. 2. Repair of the water damage in the lower level men's and women's restrooms. 3. Removal of glass door and window in upper level administration vestibule - replace with gypsum board. 4. Demolition of existing file room and construct new file room (included in 2004 CIP). 5. Paint lower and upper level lobbies, new file room, administration support office and hallway. 6. Repair water-damaged gypsum board in BuildinglLaw Enforcement stairwell. e Total Cost $25,901.00 ProDosal ReQuest No.3 This item was generated by the Architect to remove an existing fire extinguisher cabinet and patch the resulting opening. This work item was not covered in the original set of construction documents. Cost $394.00 ProDosal ReQuest No.6 This item originated from the Contractor as Request for Information (RFI) No. 11. An error in the construction documents led to the incorrect placement of a window in the new staff office location. The contractor was instructed to correct this error. Cost $278.00 These items represent $26,948 in additional construction costs. In addition to these expenses, KKE architects has recorded 59.5 hours and a total dollar amount to date of $4,395.00 in additional service hours related to Proposal Request No.2. KKE estimates that all together a total of 83.5 to 91.5 in additional service hours will be necessary to complete their work related to Proposal Request No.2. e
