Thank You Otto Bremer Foundation
Reach would like to say a big thank you to the Otto Bremer Foundation. The Otto Bremer foundation joined with
others including Hennepin County, the Bieber and the Sanner Family Foundations to support the Reach Out
program. From November 1, 2012-October 31, 2103, the Reach Out program kept 59 out of 62 referred households
away from crisis placements and safe and stable in their own community homes and apartments. We far exceeded
our goal of 80%success with a 95% rate of community stability! Without our community partners none of this would
,R
have been possible. Thank you to all who support our mission to help people with cognitive disabilities be all that
they can be.
